A Delray Beach business owner has been arrested in connection with what US authorities describe as a $3.7 billion Medicare fraud scheme — one of the largest ever uncovered in the programme’s history.

Ibrahim Khaldoon Hilmi, listed in Florida business filings as the owner of Sunshine Senior Solutions, has been brought back to the United States after being extradited from Turkey. He now faces a series of serious federal charges — including health care fraud, wire fraud conspiracy, and money laundering conspiracy.

According to the US Department of Justice, Hilmi was located in Cyprus after allegedly fleeing the United States in 2025. He was later detained and returned to US custody.

The FBI’s Miami office has released images showing his arrest last Friday.

Prosecutors say the case sits at the heart of a vast alleged fraud operation involving billions of dollars in false Medicare claims — a scheme now under intense federal scrutiny.

Hilmi is currently in federal custody as the investigation continues.
